Question 1c

Bronchial responsiveness

What is the difference between specific and non-specific bronchial responsiveness?

Your answer

  • Specific responsiveness implies bronchoconstriction when challenged with histamine or methacholine.

You have not paid enough attention.

When bronchi are challenged with stimuli of various origin, they may respond with airway narrowing. Stimuli can be divided into those which elicit an immunological response, and those in which the offending stimulus is not recognised as an allergen. The predominant immunological pathway in the airways is the type I, IgE-mediated response, which is driven by allergens (atopy). This specific response contrasts with that to non-allergic stimuli, often referred to as non-specific bronchial responsiveness. However, the terminology is becoming obsolete.
